[PATCH v3 00/21] Add new command "switch"

v3 contains document and completion updates based on v2's feedback. It also contains some extra git-checkout.txt updates (blame Eric for this, he points out problems in git-switch.txt and makes me want to go fix git-checkout.txt too).

The series is now based on 'master' (yay!)
